Causes of splenomegaly
The cause of splenomegaly depends on the underlying condition causing the disease. As noted above, splenomegaly is not by itself a disease and relies on an underlying condition or etiology. Doctors would need to perform a differential diagnosis to know which disease is causing splenomegaly. In other words, they should rule out the following causes:
1) Mononucleosis

This is a common sign in the case of mononucleosis. It is particularly common in the second and third weeks after the infection. Doctors should look for additional signs such as cervical lymphadenopathy and a sore throat in such cases. Along with splenomegaly, they are a triad that points to mononucleosis as a very likely diagnosis. These patients may also develop fluctuating body temperature with an evening fever peak, sometimes jaundice, and a skin rash.
